New Students/ Kindergarten Registration
Dear Parents and/or Guardians,
Welcome to Coolidge School! As part of the Kindergarten registration process, we would like you to be aware of the following requirements:
Your child's/children's immunizations must be up-to-date in order for him/her to start school. As required by the New Jersey Department of Health and Senior Services, documentation of immunizations must be submitted prior to the first day of school. A student will be refused entry until immunization requirements are met. Exceptions may only be granted for religious beliefs or medical recommendations.
All students entering school in the State of New Jersey must have documentation of a completed physical exam within one calendar year of their start of the school year. The physical should include hearing and vision exams, which can be completed by your physician.
For your convenience, links to the required forms are provided below. In addition to the forms, please submit a copy of your child's/children's immunization record.
