Just a reminder to you that you must pick up your child’s medication for this school year on the last day of school. It will not be sent home with your child. (if you have not picked up yet please call the main office)
Please make arrangements to see me just before or after dismissal on June 20th to take your child’s medication home. Any medication not picked up will be discarded.
Per state law, we must have new Medication Administration forms filled out by you and authorized by your child’s physician each year. The forms will be available when you pick up your child’s medication.
